Veterinary Referral Form​

This form must be completed by the referring veterinarian. 

A submitted referral is legally required before any treatment can be provided. 

Once received, Furability will contact your client to schedule an appointment.

Urgent cases will be prioritised; current wait times for new patients are approximately 4 weeks
